Forensic pathologist and chairperson of the Committee of Forensic Medicine Dr Ken Thomson tells Geoff Robinson he is concerned that hospital deaths resulting from surgical procedures are not being reported to coroners, and with the removal of medical superintendents from hospitals there are no safeguards to ensure death certificates are reliable.
